What should I expect at my initial consultation?
That you’ll be heard! You will receive a complete examination, current history, past medical history, family history, review of medicines, vitamins and supplements as well as assessment of your nutritional and lifestyle path. In a nutshell, I want to know where you’ve been, what you have done and where you want to go. If we both feel you are a good candidate for the program, we will discuss the plan to help you move forward in creating a healthier life.
How much does the Records Review and Initial Consultation visit cost?

Dr. Kaye will review the forms you fill out and any other lab reports/medical records/documents you would like him to review BEFORE  your visit. In addition, your first visit with Dr. Kaye will be about an hour and 20 minutes.   The fee for the Records Review and Initial Consultation is $375.00.

What should I bring to the initial visit?

Please bring a list of your medications, vitamins and supplements (please note the brand); any prior medical records and lab reports. Additionally, please complete the intake form 48 hours prior to your appointment.

How do I know I qualify for the Center for Functional Health Lifestyle Program?
This program isn’t for everyone. The program is about optimizing your health and requires dedication, motivation and responsibility on your part. Remember, you are part of the team to help create a healthier life for yourself and you must be willing to embrace nutritional and lifestyle changes.
Are lab tests covered?

Certain lab tests may be covered by your insurance however, when the test is NOT covered by your insurance, you will be responsible for making a payment directly to the lab. 

Do I still need my Primary Care Physician?
YES! In fact, one of the requirements of being accepted into this program is that you MUST have a primary care doctor and maintain your physicals. Your PCP is part of the team and will be receiving updated reports on your progress. Keep in mind that our goal is creating optimal health and NOT the treatment of your disease.
Do I stop taking my medications?
NO! The only individual who can tell you to stop taking your medications is the prescribing doctor. If you plan to stop taking any medication on your own, please notify the prescribing doctor.
What is the investment (time and money) for this program?

We offer customized plans to fit each individuals needs. After your comprehensive initial evaluation, the investment for Individualized plans is $900.00 for a  2 month program, and Monthly Membership of $300.00 per month there after. Functional Lab testing fees are paid directly to the lab.  Supplements are individualized and are an additional fee. Payment plans are available through Advanced Care Card. Is there a fee for missed appointments?
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. To help prevent gaps in patient care we will charge a $100.00 fee for missed inital consult visit and $75.00 fee for missed follow-up visits.
Does your office accept insurance for these services?
We do not accept insurance or Medicare for these consulting services. However, we will provide you with a Superbill so you can submit it to your insurance provider for out-of-network benefits. You may be able to use your FSA/HSA however, please first check with human resources.
What happens if I want to utilize your chiropractic services?

Chiropractic and musculo-skeletal rehabilitation services can be provided; however, these services are SEPARATE from functional health programs. Therefore, you may use your insurance if you have chiropractic benefits (all co-pays are required at the time of treatment) or choose to be a cash patient.

Are lab tests included?

Some of our programs include testing. If your program does not, you will be responsible to pay the lab for your testing directly at direct lab pricing.

How much are lab costs?

Please budget $500 for functional lab tests; however, some of your lab tests may be covered by insurance.

How does lab testing work?
Bloodwork is performed at your insurance providor’s preferred laboratory (usually Lab Corp or Quest) and sometimes a phlebotomist (the person who draws your blood) can come to your home to draw blood. Additional functional lab tests such as stool or urine are done in the comfort of your own home. We provide you with a kit and clear instructions. Again, payment is made directly to the lab.
Does your office accept patients that live in other states?

Dr. Kaye accepts patients from Pennsylvania, California and Florida.
