Healthspan: Why How Well You Live Matters as Much as How Long

For decades, the conversation around aging has focused on one thing: living longer. Advances in medicine, public health, and technology have helped people live more years than ever before. While that’s certainly something to celebrate, it’s only part of the story.

The real question isn’t just How long will I live?

It’s How well will I live?

That’s where the idea of healthspan comes in.

In my work with patients at The Center for Functional Health, this is a conversation I have almost every day. People come to me feeling exhausted, frustrated, and stuck. They’ve been told their labs are “normal,” but they know something isn’t right. They don’t just want to live longer. They want to feel strong, clear-headed, and energized for the years ahead. That desire is really what healthspan is all about.

The concept builds on James Fries’ influential “compression of morbidity” hypothesis, which proposed that the onset of chronic illness could be delayed so that a greater proportion of life is spent in good health (Fries, 1980). Instead of measuring success by the number of birthdays we celebrate, it encourages us to focus on maintaining our energy, strength, independence, and overall quality of life for as many years as possible.

What Is the Difference Between Lifespan and Healthspan?

Although the terms are often used together, they describe two very different things.

Lifespan is the total number of years you live.

Healthspan is the number of those years you spend in good health, free from significant chronic disease or disability.

Imagine two people who both live to be 90 years old. One remains active, travels, gardens, exercises regularly, and enjoys life well into their eighties before experiencing a relatively short period of declining health. The other develops diabetes, heart disease, arthritis, and chronic fatigue in their fifties and spends decades managing multiple health conditions.

Both individuals have the same lifespan. But their healthspan is dramatically different.

I shared a story in a recent blog post about a 74-year-old patient I saw about a decade ago. Aside from mild back pain, her exam was great. Her blood pressure, her pulse, her energy. She was thriving. That’s what a long healthspan actually looks like in practice, and it’s something I want for every one of my patients.

According to the World Health Organization, evidence suggests that the proportion of life spent in good health has remained broadly constant even as life expectancy has increased, meaning that many of the additional years people are gaining are spent in poor health (WHO, 2024). That finding is why healthspan has become one of the most important conversations in modern healthcare.

Why Healthspan Matters

Living longer is a wonderful goal, but living well is what most people truly hope for.

As we age, our risk of developing chronic conditions naturally increases. At the biological level, aging results from the accumulation of molecular and cellular damage over time, leading to a gradual decrease in physical and mental capacity and a growing risk of disease (López-Otín et al., 2023). Common conditions in older age include heart disease, diabetes, cognitive decline, osteoarthritis, chronic obstructive pulmonary disease, depression, and dementia (WHO, 2024).

However, these changes are neither linear nor consistent, and they are only loosely associated with a person’s age in years. As the WHO notes, some 80-year-olds have physical and mental capacities similar to many 30-year-olds, while other people experience significant declines much younger. There is no typical older person.

This is something I see firsthand in my practice. Two patients the same age can have completely different health pictures depending on their metabolic function, their lifestyle history, their stress load, their gut health, and the environments they’ve lived in over the course of their lives.

Focusing on healthspan shifts our mindset from reacting to illness to proactively supporting long-term wellness, through both individual choices and the environments and communities we live in.

The Role of Environment, Community, and Social Connection

While individual habits matter enormously, healthy aging doesn’t happen in a vacuum. The WHO emphasizes that although some variation in older people’s health is genetic, most is due to physical and social environments, including homes, neighborhoods, and communities, as well as personal characteristics such as sex, ethnicity, and socioeconomic status. The environments people live in as children, or even as developing fetuses, have long-term effects on how they age (WHO, 2024).

Supportive environments, such as safe and accessible public spaces, walkable neighborhoods, reliable transportation, and age-friendly community design, enable people to stay active, engaged, and independent even as their capacities change. Conversely, environments marked by poverty, isolation, pollution, or limited access to healthcare can accelerate decline regardless of personal effort.

Social connection deserves special attention. A landmark meta-analysis found that strong social relationships are associated with a 50% increased likelihood of survival, making social isolation a mortality risk factor comparable in magnitude to smoking, excessive alcohol consumption, and physical inactivity, and exceeding the risk associated with obesity (Holt-Lunstad et al., 2010). More recent work continues to reinforce these findings: a 2023 WHO commission on social connection concluded that loneliness and social isolation are significant public health concerns affecting people of all ages, with wide-ranging effects on cardiovascular health, immune function, mental health, and cognitive decline (WHO Commission on Social Connection, 2023).

I bring this up with my patients regularly, because it’s easy to focus entirely on labs and supplements and meal plans while overlooking the fact that your relationships, your sense of purpose, and your connection to community are just as important. Maintaining meaningful relationships, participating in community life, and fostering intergenerational connection are not luxuries or afterthoughts. They are foundational elements of a long and healthy life.

Nutrition: Building the Foundation for Healthy Aging

A nutrient-dense eating pattern rich in vegetables, fruits, quality protein, healthy fats, legumes, nuts, seeds, and fiber provides the building blocks for healthy muscles, bones, hormones, and immune function. Research on dietary patterns such as the Mediterranean diet has demonstrated meaningful associations with reduced cardiovascular risk, improved metabolic health, and lower rates of chronic disease (Martínez-González et al., 2019). Over time, these everyday choices can help reduce the risk of chronic disease while supporting energy, resilience, and healthy aging.

At the same time, one of the most common patterns I see in my practice is chronic undereating. Many of my patients come to me assuming they need to eat less, when in reality their bodies are under-fueled. As I wrote about recently, eating too little can hold back your health goals just as much as eating poorly. The key is finding the right balance for your body, your activity level, and your goals.

Nutrition also plays a key role in blood sugar balance, which is closely linked to long-term health. Frequent blood sugar spikes and poor insulin sensitivity can contribute to inflammation, weight gain, fatigue, and increase the risk of conditions such as type 2 diabetes and cardiovascular disease. Building balanced meals with protein, healthy fats, fiber, and complex carbohydrates helps support more stable energy levels and metabolic health.

Another important piece of the puzzle is gut health, and this is an area I’m especially passionate about. A healthy digestive system allows you to properly digest food, absorb nutrients, and support a diverse gut microbiome. Emerging research suggests that the gut microbiome influences not only digestion but also immune function, metabolism, and potentially aspects of brain health through the gut-brain axis, though much of this research is still evolving and many findings have yet to be fully confirmed in large human trials (Cryan et al., 2019).

Good digestion also depends on regular elimination, an often-overlooked part of overall wellness. Adequate fiber, hydration, and a varied whole-food diet help support healthy bowel function and the body’s natural ability to eliminate waste products.

Your body is also equipped with highly effective detoxification systems. The liver, kidneys, digestive tract, lungs, and skin work continuously to process and remove waste and environmental compounds. Rather than relying on restrictive cleanses or detox programs, supporting these natural pathways through proper nutrition, hydration, movement, and healthy digestion allows your body to do what it was designed to do.

Metabolic Health: One of the Biggest Predictors of Healthspan

Many of the chronic diseases associated with aging have one thing in common: they often begin with changes in metabolic health.

Metabolic health refers to how efficiently your body produces and uses energy. It includes blood sugar regulation, insulin sensitivity, cholesterol and triglyceride levels, blood pressure, inflammation, and body composition. When these systems are functioning well together, they support steady energy, cardiovascular health, hormone balance, brain function, and healthy aging.

This is one of the core areas we focus on at The Center for Functional Health. I see patients every week whose metabolic health has been quietly declining for years, long before anything shows up on a standard screening. Insulin resistance, early blood sugar dysregulation, and inflammatory changes can develop slowly and silently. By the time someone receives a diagnosis of prediabetes, type 2 diabetes, or cardiovascular disease, those changes have often been building for a long time.

If you’ve read my recent blog post on cholesterol, you know that even something as familiar as a lipid panel tells a much more nuanced story than most people realize. The goal is to look deeper, identify concerns early, and make meaningful changes before more significant problems develop.

Movement, Strength, and Muscle: One of the Best Investments in Healthy Aging

Exercise is one of the most powerful tools we have to support healthspan. A comprehensive review of the evidence concluded that regular physical activity reduces the risk of premature death and is associated with the primary and secondary prevention of numerous chronic diseases, including cardiovascular disease, diabetes, cancer, hypertension, obesity, depression, and osteoporosis (Warburton et al., 2006). More recent evidence from large-scale studies continues to confirm that even modest increases in physical activity are associated with meaningful reductions in mortality and disease risk (Ekelund et al., 2019).

While all movement is beneficial, strength training deserves special attention. As we age, we naturally experience a gradual loss of muscle mass and function, a process known as sarcopenia. Research suggests that clinically meaningful muscle loss typically becomes apparent around the fourth decade and accelerates after age 60, contributing to reduced mobility, decreased strength, increased fall risk, loss of independence, and a slower metabolism (Cruz-Jentoft et al., 2019).

Muscle is much more than something that helps us move. Skeletal muscle is the body’s largest site of insulin-mediated glucose disposal and plays an important role in metabolic regulation, bone health, balance, and overall resilience (Wolfe, 2006).

I tell my patients that the goal isn’t to look like an athlete. The goal is to preserve the strength and function that allow you to keep hiking, carry your groceries, travel, play with your grandchildren, or simply get up from a chair with confidence. Those daily capabilities are what healthspan looks like in real life.

Avoiding Tobacco: A Non-Negotiable for Healthy Aging

No discussion of healthspan is complete without addressing tobacco use. The WHO identifies refraining from tobacco as one of the most important modifiable behaviors for reducing the risk of non-communicable diseases, improving physical and mental capacity, and delaying care dependency (WHO, 2024).

Smoking accelerates nearly every hallmark of biological aging. It increases the risk of cardiovascular disease, chronic obstructive pulmonary disease, numerous cancers, and cognitive decline. It damages blood vessels, impairs lung function, weakens the immune system, and accelerates the loss of bone density and muscle mass. The good news is that quitting at any age produces measurable health benefits, and the earlier a person stops, the greater the gains in both lifespan and healthspan.

Brain Health and Mental Well-Being: Protecting Your Cognitive Future

Healthy aging isn’t just about keeping your body strong. It’s also about protecting your brain.

Cognitive health is influenced by many of the same lifestyle habits that support the rest of the body. Regular exercise, nutritious food, healthy blood sugar regulation, quality sleep, social connection, and lifelong learning all contribute to maintaining brain function as we age. The WHO’s guidelines on dementia risk reduction highlight that a significant proportion of dementia risk, potentially up to 45%, is attributable to modifiable factors across the life course, including physical inactivity, tobacco use, excessive alcohol consumption, social isolation, and poorly managed cardiovascular risk factors (WHO, 2024).

Mental well-being is equally important. Chronic stress, anxiety, depression, and poor sleep can affect memory, concentration, mood, and overall quality of life. Over time, they may also influence physical health through changes in inflammation, hormone regulation, and immune function.

I encourage my patients to think about cognitive health the same way they think about physical fitness: it’s something you build and maintain over time. Staying physically active, challenging your mind with new skills, maintaining meaningful relationships, spending time outdoors, prioritizing restorative sleep, and managing stress in healthy ways all help support cognitive resilience and emotional well-being throughout life.

Sleep and Recovery: Where Your Body Repairs and Rebuilds

Sleep is often one of the first things we sacrifice when life gets busy, yet it’s one of the most important investments we can make in our long-term health.

While you sleep, your body is hard at work repairing tissues, regulating hormones, consolidating memories, supporting immune function, and recovering from the physical and mental demands of the day. Research on the glymphatic system suggests that sleep also plays a role in clearing metabolic waste products from the brain, though this is an active area of investigation and our understanding continues to evolve (Xie et al., 2013).

A comprehensive review of the health consequences of sleep disruption found that consistently poor sleep is associated with an increased risk of obesity, type 2 diabetes, cardiovascular disease, depression, and cognitive decline, while adequate quality sleep supports healthier metabolic regulation, immune function, mood, and cognitive performance (Medic et al., 2017).

Stress and sleep are also closely connected. Chronic stress can make it difficult to fall asleep or stay asleep, while poor sleep can increase stress levels, creating a cycle that affects both physical and mental well-being. Many of my patients are caught in exactly this cycle, and it’s one of the first things we work on together.

Prioritizing healthy sleep habits, such as maintaining a consistent sleep schedule, limiting screen time before bed, creating a relaxing bedtime routine, and addressing underlying issues that may be disrupting sleep, can have a meaningful impact on your overall healthspan.
